This video will provide helpful information to complete the PHS. The examiner will discuss the applicant's understanding of the content of the video during the in-office interview.
Step 3
Complete the Personal History Statement using the tools, definitions and topic descriptions provided in the video.
WE DO NOT RECEIVE ANY APPLICATIONS, PIQ'S, PHS's OR ANY OTHER PAPERWORK FROM THE AGENCY YOU HAVE APPLIED TO.
DO NOT ASSUME WE ALREADY HAVE INFORMATION ON YOUR BACKGROUND WHEN FILLING OUT THIS PAPERWORK.
